Formdata to textfile

Dear stackoverflow community I have a question to ask of you.

I would like to create a form with multiple insert strings and the use of radiobuttons/checkboxes.

The intention is to make a form my dad can easily fill out to get a neat text he can put on a dutch site(kind of a craiglist website).

i.e.

Type:[Insert type here]

Size:[Insert size here]

Which features does it have: [ ]Feature A [ ]Feature B [ ]Feature C

Other features:[Insert features here]

And the output should look like 'A neat looking [Type] with size [Size] with >[Chosen features] (and [Other features]).

I have been trying to get it working for three days now and none of the codes i tried actually return anything. I feel like I am running against a very solid brick wall.
