Personalize google Drive Forms

I've been asked to modify the style of some questionnaires created by using Google Drive Forms. I need to have a better control of field's dimensions, and add specific fields (ie: subtitle).

If this were acutally possible by using JavaScript, can anyone indicate me a basic tutorial which can teach me how to insert JavascriptCode in google Drive Forms.
