Select insert form in ApEx

I have page ,where I have one select list, which is placed in Region area(Body1) and it serves as a selector of insert forms. In anothers bodies (Body2 and Body3) I have two form which serve as insert form for data in tables. Then I have one dynamic action which serve to show/hide the form for insert. The problem is when I select one of the form and type data in form and then click on CREATE button, it throw this:

1 error has occurred ORA-06550: line 1, column 151: PL/SQL: ORA-00957: duplicate column name ORA-06550: line 1, column 13: PL/SQL: SQL Statement ignored

Maybe the problem is that I have two forms on one page.

Can anyone help how to fix ? Thanks
