Drupal update from 7.17 to 7.34 broke user registration form

I have a Drupal site that when we updated from v7.17 to v7.34, there is a 30-day trial registration form that broke in 2 ways. I believe this form is based off of Drupal's standard user registration form:

1. I am missing one of the form buttons (Sign up for a full subscription).


2. When I submit the "new" form, I get an error saying the username filed is required, and the specified passwords do not match. As a note, there is only 1 password field and no username field on the form.

I have looked around online, and seen comments regarding honeypot, but that does not appear to b causing my problem. Any suggestions regarding how Drupal composes and processes forms, and what may be causing this error would be greatly appreciated.
