Pass Data Form Info

I have a little and complicated question again.

Im in need of a script which the gathered / input data pass to the other form.

example: www.mysite.com/form

(pass the gathered data from www.mysite.com)

to

http://ift.tt/1HAuJYc

is that possible?

**Note: I own www.mysite.com I don't own www.somewebsite.com

or if there any other way to do that? like url trick,prefill form.

please help me.
