How to send a custom form?

I've got set up a form with a couple of questions. I also filled a database with a set of name + emails to which the form has to be sent to. I also got the proper links and the form gets a bit more personal by taking out values of the database so I can say "Hello [name]", instead of a general form.

But now I want to send this form to all the people in the database. But how do I do that? Do I generate an URL for every record in the database? Or do I create these URLS on the fly?
